- I need a confirmation of insurance for the embassy. Can it be issued for me?

For this purpose, an insurance policy is used, which contains a receipt of payment. If you have any problems receiving the insurance policy, please contact maxima.poistovna@gmail.com. Both the Comprehensive Alien Health Insurance and the Necessary and Urgent Care Insurance also meet all the conditions for a residence permit. Having an insurance policy in place is a prerequisite for being granted a visa. The insurance meets the conditions laid down in Act No 404/2011 Coll. on the residence of foreigners and on amending and supplementing certain acts on the territory of the Slovak Republic.

For policies arranged online, premiums can only be paid by credit card. When arranging insurance at partner branches, you must pay by bank transfer. If you have been treated at a Maxima contracted medical facility, this facility will invoice the insurance company directly - you will not be reimbursed for the costs (except for dental treatment), so we recommend that clients only use contracted medical facilities. If you have visited another medical facility and have paid for the treatment in cash, you must send the insurance company: the original proof of payment, the medical report, the bank account number and the completed Application for Reimbursement of Foreigners' Health Insurance form. These documents should then be sent to info@maxima-as.cz or to the address of the insurer: Maxima pojišt'ovna a.s., Liquidation of insurance claims, Italská 1583/24, 120 00 Praha 2 - Vinohrady, or via your intermediary.
